In this role, you will serve as the liaison between physician practices and various stakeholders to drive value-based care initiatives and improve clinical outcomes. Why Join DaVita IKC? Key Responsibilities: Physician Engagement: Build and manage relationships with nephrologists and other key stakeholders to drive trust and satisfaction (70% of the time). Stakeholder Collaboration: Develop formal relationships with stakeholders within the local integrated network to ensure program success. Education and Training: Educate practices on program requirements and facilitate engagement across value-based care programs. Process Improvement: Implement meaningful process changes to optimize clinical and patient outcomes, leveraging data analysis and best practices. Team Leadership: Directly supervise a regional team of Care Coordinators, providing oversight of day to day operations and guidance navigating insurance benefits, community resources and support services (30% of the time.) Qualifications: Bachelor's Degree required; MBA preferred. Proven track record of building successful physician relationships. Process improvement training (Lean Six Sigma, PDSA) preferred. At least five years of experience in a clinical or clinical support role. Advanced proficiency in Microsoft Office Products and strong analytical skills. At least 2 years of project management experience or supervisory experience. Prior experience in healthcare services and/or working with value-based care models.
