DescriptionWe are offering a PART TIME contract to permanent employment opportunity for a Patient Care Coordinator. This role is in the healthcare industry and requires a dedicated detail oriented to manage and coordinate patient care in a healthcare facility. The Patient Care Coordinator will be a crucial link between patients, their families, and the healthcare team, ensuring an optimal experience for all parties.
Responsibilities:
  • Manage patient intake and discharge processes to ensure smooth transitions in care
  • Act as the facilitator for communication between patients, family members, medical staff, and administrators
  • Monitor patient schedules, appointment times, and transportation needs proactively
  • Respond to patient concerns or questions, escalating complex matters to appropriate staff as needed
  • Collaborate with healthcare professionals to establish an effective patient care plan
  • Uphold patient confidentiality at all times and follow all regulations regarding patient health information
  • Oversee and document the details of patients' medical histories, condition progress, and care plans
  • Assist patients and their families in understanding the treatment plan and managing expectations of care
  • Ensure all care facilities are adequately stocked with necessary supplies
  • Carry out administrative duties such as maintaining patient records, scheduling appointments, and arranging for laboratory services
  • Verify insurance details and manage patient appointments with the use of basic office skills and administrative office tools
Requirements
  • Minimum of 2 years of experience in a patient care coordination role or similar healthcare environment
  • Proficiency in verifying insurance coverage details and explaining them to patients
  • Demonstrated ability to schedule and manage patient appointments effectively
  • Basic office skills including filing, typing, and handling telephone calls
  • Previous experience in an administrative role within a healthcare setting is preferred
  • Excellent interpersonal skills for interacting with patients, healthcare professionals, and insurance providers
  • Strong organizational skills and attention to detail
  • Ability to handle confidential patient information with discretion
  • Proficiency with standard office software and equipment
  • Good problem-solving skills and ability to handle stressful situations calmly
  • Familiarity with medical terminology and healthcare procedures
  • Excellent written and verbal communication skills
  • Ability to work independently and as part of a team.
