Responsibilities:
- Manage patient intake and discharge processes to ensure smooth transitions in care
- Act as the facilitator for communication between patients, family members, medical staff, and administrators
- Monitor patient schedules, appointment times, and transportation needs proactively
- Respond to patient concerns or questions, escalating complex matters to appropriate staff as needed
- Collaborate with healthcare professionals to establish an effective patient care plan
- Uphold patient confidentiality at all times and follow all regulations regarding patient health information
- Oversee and document the details of patients' medical histories, condition progress, and care plans
- Assist patients and their families in understanding the treatment plan and managing expectations of care
- Ensure all care facilities are adequately stocked with necessary supplies
- Carry out administrative duties such as maintaining patient records, scheduling appointments, and arranging for laboratory services
- Verify insurance details and manage patient appointments with the use of basic office skills and administrative office tools
- Minimum of 2 years of experience in a patient care coordination role or similar healthcare environment
- Proficiency in verifying insurance coverage details and explaining them to patients
- Demonstrated ability to schedule and manage patient appointments effectively
- Basic office skills including filing, typing, and handling telephone calls
- Previous experience in an administrative role within a healthcare setting is preferred
- Excellent interpersonal skills for interacting with patients, healthcare professionals, and insurance providers
- Strong organizational skills and attention to detail
- Ability to handle confidential patient information with discretion
- Proficiency with standard office software and equipment
- Good problem-solving skills and ability to handle stressful situations calmly
- Familiarity with medical terminology and healthcare procedures
- Excellent written and verbal communication skills
- Ability to work independently and as part of a team.
