JOB This position is a member of a multi-disciplinary team serving individuals with serious and persistent mental illness and/or co-occurring substance use disorders. The position provides virtual, office, and community based case management services intended to maintain functioning and facilitate recovery. Supports and promotes the Mission, Vision, and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co-Workers." 50% Provide Direct ServicesManage a caseload of individuals who are receiving prescriber services with Rock County Counseling Center.Provides various assessmentsParticipates with recovery teams as assignedEngage in check ins and reminder calls as indicated to monitor stability and increase prescribe appointment attendance, including providing or arranging transportation, follow up calls, and letters if appointments are missedProvides medication monitoring supportProvides advocacy for identified clientsProvides linkage and support for service referralsProvides crisis intervention as neededAttend prescriber appointments when indicated25% Provide Service Facilitation functionsFacilitates the assessment processCollaborate with natural supports, internal and external partnersFacilitates the development of the service plan as well as ongoing monitoring and review20% - Miscellaneous dutiesEnsures compliance of all programs and medical records Attends weekly staffing meetings with each prescriber as neededAttends all program/department meetings as requiredComplete 100% Staff Time Entry5% - Performs other duties as assigned EXAMPLE OF DUTIES Professional expertise in the following areas preferred. Training and ongoing professional development will be provided. Knowledge of the principles and practices of Social Work and its applicationKnowledge of laws, regulations, and practices pertaining to federal and state human service programsKnowledge of assessment and treatment of mental illness and substance use disordersKnowledge of psychotropic medications, their main effects, side effects, and abuse potentialKnowledge of psycho-social rehabilitation servicesKnowledge of State and Federal entitlementsAbility to maintain accurate records and prepare clear and concise reportsAbility to establish and maintain effectively working relationships within the agency and communityAbility to understand and carry out verbal and written instructionsAbility to follow all departmental as well as divisional policies and procedures SUPPLEMENTAL INFORMATION Physical elements: Ability to travel throughout the community.Potential for interactions with animals including but not limited to household pets.Must be willing to transport clients Potential exposure to challenging and unpredictable environments.Participate in disaster evacuations and fire drills when circumstances warrant. Maintain safety procedures.Lifting and carrying up to 25lbs on occasion.Equipment use: Ability to use a computer and keyboard.Must become efficient at using a computerized documentation within 2 months of hire including the use of Microsoft Office products, an electronic medical record system, and other web based systems.Ability to use a copy and fax machine as well as a cellphone.Working conditions: Able to work in multiple locations including the office, client's homes, and a variety of community locations.Available to work a flexible schedule including evenings when needed to coordinate client emergencies, care, and recovery plan services.Potential exposure to physically or verbally abusive persons.
