​​​​​Join our team as a Care Coordinator, where you'll be the crucial link between patients and their care journey. As a patient advocate, you'll guide individuals from the moment they're recommended for care, through their treatment, and 90 days beyond discharge, ensuring they receive the highest level of support.

Key Responsibilities:

Program Coordination:

  • Support patients and families throughout their care experience.
  • Provide clear information about programs and services.
  • Coordinate admissions and necessary paperwork.
  • Address patient and family inquiries.
  • Maintain regular communication with patients on waiting lists.
  • Coordinate COVID testing for residential patients.
  • Verify insurance and collaborate with Patient Financial Services.
  • Maintain a clear visual of each patient's progress from recommendation to discharge and beyond.

Patient Support:

  • Maintain open communication with patients.
  • Ensure seamless care from program to discharge and beyond.

Interdisciplinary Collaboration:

  • Develop strong relationships with various team members to facilitate smooth communication about patient care.
  • Coordinate with departments including Admissions, Patient Access, Providers, Outpatient operations, and others.

Schedule:

  • Monday to Friday, 8:30 am to 5:00 pm - Perfect for work life balance!

Requirements:

  • Bachelor’s degree preferred; High School diploma required.
  • Two years of customer service experience, preferably in healthcare.
  • Proficiency in computer software and EMRs.
  • Knowledge of medical/psychiatric/chemical dependency treatment processes and managed care preferred.
  • Valid driver's license preferred for Oconomowoc location.
