PUBLIC HEALTH CARE MANAGER I/II Job #847-1

Located in the heart of North Carolina’s Piedmont, Rowan County is attractively and conveniently nestled between the state’s largest city and its Capitol. The County is currently accepting applications for the position of Public Health Care Manager I/II for CMHRP (Care Management for High Risk Pregnancies). This position provides care coordination for Medicaid eligible and uninsured pregnant and postpartum women; assesses and develops care plans based on the need of the patient; implements health education, nutrition, and psycho-social interventions; manages referral activities; and conducts visits with the patient in a variety of settings which include the home, community, office and/or medical office.

Requires considerable knowledge of social work principles, techniques, and practices and their application to specific casework, group work, and community problems; knowledge of psychosocial, socioeconomic, and behavioral problems and their treatment; knowledge of governmental and private organizations and community resources; and skill in establishing rapport with clients and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client’s problems.

Minimum qualifications include a Bachelor's Degree in Social Work; or Bachelor’s Degree in related Human Services degree plus one year of directly related experience for the PHCM I; PHCM II requires: Bachelor’s Degree in Nursing which includes a Public Health Nursing rotation and one year of professional nursing experience, preferably in Public Health; or graduation from a school of professional nursing and two years of nursing experience, preferably with one year of experience in Public Health; or an equivalent combination of education and experience. Degree/Coursework must be from an appropriately accredited institution. Dependent on degree a current RN license with the North Carolina Board of Nursing may be required; valid driver’s license required.
