What You’ll Do:

​

As a Nurse Practitioner Supervisor (NPS), you’ll play a pivotal role in ensuring the well-being and independence of our participants. Your responsibilities will include:

• Supervising and guiding Nurse Practitioners and clinical staff to deliver exceptional care.
• Conducting thorough wellness assessments and providing urgent care services.
• Coordinating smooth transitions for participants requiring hospital care and managing complex cases.
• Engaging in collaborative Interdisciplinary Team meetings and contributing to ongoing care management education.
• Ensuring compliance with HIPAA regulations and fostering clinical documentation improvement.
• Offering home visits and emergency coverage when necessary, ensuring continuity of care.

What You Bring:

We’re looking for candidates who possess:

• Graduation from a nurse practitioner education program.
• A minimum of two years of clinical nursing experience.
• Licensure as a Nurse Practitioner with DEA Certification and Board Certification.
• Proficiency in more than one language is preferred.
