Rush University Medical Center is currently seeking a full-time, Primary Care Advanced Practice Provider to join the Primary Care team at Earvolino and Associates, an outpatient practice located at Rush's main campus in downtown Chicago!
This role will be primarily involved with patient evaluation and management and chronic care management. The Advanced Practice Provider (APP) selected for this role will support the Department of Internal Medicine, Division of General Internal Medicine, partnering with a group of primary care physicians in the practice to assist in managing a panel of patients, functioning collaboratively in support of a team-based format for care delivery that addresses all acute, chronic, and health maintenance needs. The APP should demonstrate competency in the diagnosis, treatment and management of acute and chronic diseases, while emphasizing health promotion and disease prevention. The ideal candidate will have APP experience in Primary Care.
The Advanced Practice Provider functions as a member of the health care team to provide high quality, comprehensive acute and/or routine medical/surgical care of Rush University Medical Center patients within the scope of practice granted in accordance with the RUMC medical staff bylaws. Promotes collaboration between multiple disciplines to ensure cost effective and quality patient care. Successful candidates will also meet all other state and federal regulatory practice and accreditation standards. Exemplifies the Rush mission, vision and values and acts in accordance with the policies, procedures, guidelines of Rush, including the Rush University Medical Group Customer Service standards.
