Job Summary:

The Advanced Practice Provider, Left Ventricular Assist Device (LVAD) Coordinator is primarily responsible for providing evidenced-based care for patients on mechanical circulatory support. The role encompasses specialized care focused on protection, promotion, and optimization of the health and abilities of the patient with heart failure as well as the VAD recipient. The position is responsible for working with the interdisciplinary team to participate in decision making regarding patient eligibility for advanced therapies, coordinate care prior to VAD implant, and provide complex care management in both the inpatient acute care and ambulatory settings. Serves as a professional resource to other members of the health care team and supports the strategic goals of the Advanced Heart Failure (AHF) program. This position requires a high degree of clinical collaboration across the RWJBH enterprise and community to provide high quality care and patient care solutions across the continuum.

Qualifications:

• Valid NJ RN and APN license (or Physician Assistant (PA) license)
• Two years of experience in caring for patients on mechanical circulatory support is preferred
• ACLS and BLS required

• Full Time
• Day Shift
• 37.5 Hours

Essential Functions:

• Evaluate VAD recipients and potential recipients by obtaining personal and medical history & medical history, reviewing diagnostic and laboratory test results, and scheduling medical/psychological consultations as warranted.
• Assist physicians with medical treatment, and provide plan of care information to patients and/or family members. Schedule clinic visits, labs, diagnostic studies, admissions, and emergency visits. Provide telephone triage of patient related concerns, reporting to medical providers with patient status/condition changes as warranted.
• Provide on-call, Heart Failure/VAD related coverage to address the needs of patients, hospital staff and outside organizations. Act as the liaison with VAD companies.
• Participate in intra and interdisciplinary programs and committees, attend organ selection committee meetings, quality assurance performance improvement (QAPI) review meetings, departmental education meetings, and participate in hospital education to staff as warranted which may include annual competency training and/or initial orientation.
• Navigate, edit, and maintain the electronic medical records and regulatory electronic data submission elements. Compile, calculate, and analyze statistical, medical, and operational data to report to government, regulatory, and medical organizations. Ensure the accuracy and completeness of regulatory required care plans and organ sharing forms (if indicated)
• Track quality data indicators and provide on time monthly reporting for department QA and for Intermacs
• Collaborates with perfusionists and assistance with set up of new implants in the operating room. Assist staff during surgical cases to provide guidance and/or trouble shooting as necessary
• Maintain and deliver necessary VAD equipment to locations within the institution as needed for patient safety. Track annual preventative maintenance with Biomed ensuring accurate
