Job Summary:
The Advanced Practice Provider, Left Ventricular Assist Device (LVAD) Coordinator is primarily responsible for providing evidenced-based care for patients on mechanical circulatory support. The role encompasses specialized care focused on protection, promotion, and optimization of the health and abilities of the patient with heart failure as well as the VAD recipient. The position is responsible for working with the interdisciplinary team to participate in decision making regarding patient eligibility for advanced therapies, coordinate care prior to VAD implant, and provide complex care management in both the inpatient acute care and ambulatory settings. Serves as a professional resource to other members of the health care team and supports the strategic goals of the Advanced Heart Failure (AHF) program. This position requires a high degree of clinical collaboration across the RWJBH enterprise and community to provide high quality care and patient care solutions across the continuum.
