We have a rare and exciting role for an RN in our Case Management department. You get to work as a team with a Social Worker to provide supportive client centered HIV/AIDS case management services (AIDS Medi-Cal Waiver Program) in San Joaquin and Stanislaus counties.
Ideal for:
RNs seeking:
· Patient-centered role-Minimal physical demands. (Mostly vitals, questions, and charting.)
· Ideal daytime hours (M-F Days), no shift work, no rush from patient to patient (our patients are clients).
· Friendly office environment and (when COVID restrictions are lifted) visit client homes with social worker.
Requirements:
· CA Licensed RN, 2 years minimum, 1 year in community service role preferred.
· Able to work in office environment initially and eventually go to client homes. (Remote work is not an option.)
· Thrive on helping a diverse group of clients in their home setting, good people skills a must.
· Be able to help clients remain connected to appropriate medical/mental health/AOD/community services with a goal of stabilizing their health, meeting their needs, and avoiding higher levels of care.
· Experience providing similar or related services and/or working with this population preferred but we can train the right candidate.
