JOB DESCRIPTION: LICENSED VOCATIONAL NURSE - CLINICAL SUPPORT STAFF

REPORTS TO: RN Supervisor/Director of Patient Care Services

JOB DUTIES AND RESPONSIBILITIES

1. Assists in the assessment of patient's need, plan, implements and documents nursing care under the direction of RN Supervisor or Director of Nursing

2. Performs tasks based on the Nurse Practice Act; maintains agency compliance with Federal and State regulations.

3. Informs the physician and family members of the changes in patient's condition and needs.

4. Under the direction of the RN Supervisor, LVN promotes education for patient's families and patients; and participates in Case Conferences; participates in revising plan of treatment, problem list and/or care plan; assists in ordering supplies and equipment needed by the patient as requested by PCG or shift LVN.

5. Completes incident reports as needed per agency policy.

6. Maintains familiarity with case load's special routine for the patient, to facilitate care for patient and better relationship with patient's family; assists the N by visiting patients to do regular "check-ins" to ensure all patient's needs are adequately met by agency.

7. Reviews Emergency Plan with family at least annually together with the RN Supervisor.

8. Assists with last minute staffing shortages on a case by case basis, as requested by PC during instances when they absolutely need an LVN on shift.

9. When providing LVN patient care coverage: Follow the Plan of Treatment as ordered by the physician. Give complete report of patient to incoming personnel to ensure continuity of care e.g., condition, intake output, prn medication, and unusual symptoms. Promote patient safety at all times. Administer GT feeding via pump/bolus/gravity feeding. Administer medication, monitor for adverse reaction. Notify MD for any abnormal findings and deviations from baseline. Implement new MD orders. Complete all necessary charting on each worked shift. Accompanies patient on MD appointment as needed. Clean all equipment used on your shift and tidy patient environment.

10. Coordinates with shift LVNs If they are short of forms and PPEs; receives report from shift LVNs regarding new orders to be transcribed in the EMR; assists and instructs shift LVNs who are transitioning to electronic charting.

11. Communicates and maintains a good relationship with MD offices to ensure POCs and orders are signed in a timely manner; requests needed documents from MD office for TAR purposes; occasionally markets to MD offices to introduce and promote services offered by ASC.

12. Collates TAR documents and coordinates with appropriate office staff to ensure TARs are submitted on a timely basis.

13. Assists the Recruitment team regarding clinical aspects of matching a suitable LVN with a new referral.
