Safeway Home Healthcare is Hiring a 245 D and Personal Care Assistance Program Director Near Roseville, MN

Description

We are seeking a 245D and Personal Care Assistance Program Director to oversee personnel management, client services coordination, program development, and financial performance. The ideal candidate will possess strong leadership abilities, excellent communication skills, and a comprehensive understanding of the needs of individuals with disabilities or seniors.

Responsibilities:

Personnel Management:

• Support staff growth through performance management and training.

• Manage hiring, work assignments, and staff evaluations.

• Ensure staffing levels and budget adherence.

Client Services Coordination:

• Screen referrals, conduct intakes, and develop service contracts.

• Create and review individualized service plans.

• Investigate and address reports of neglect or abuse.

• Oversee client programming and liaise with guardians and service providers.

Program Development:

• Align programs with organizational mission.

• Develop and maintain policies, procedures, and community relationships.

• Conduct program evaluations and participate in quality improvement.

Financial Performance:

• Create rate data and manage financial metrics.

• Ensure fiscal integrity through budget and financial reporting.

Requirements

  • Meet Designated Manager qualifications per 245D requirements.
  • 3 years of supervisory experience in direct support services.
  • Relevant educational qualifications (degree, diploma, or training with experience).

Skills/Abilities:

  • Leadership and accountability.
  • Effective time management and decision-making.
  • Experience supporting vulnerable individuals.
  • Problem-solving and conflict resolution skills.

Working Conditions:

  • Use of standard office equipment.
  • Sitting or standing for long periods, occasional lifting up to 50 pounds.
  • Driving within the Twin-Cities area
