Full Time 3rd Shift  40 per week 2-12 hour days and 2-8 hour days with alternating weekends

Wage: $17.42 an hour

Position Summary:

Under the supervision of Resident Care Manager, provide care for resident who can no longer live independently. Supervise, monitor and assist the residents ADLs (Activities of Daily Living) and ensure the residents health and well-being. Perform administration and supervision of medication or personal care services. Oversee the daily activities performed by the Certified Aides and the Resident Service Aides. All Andahwod staff must adhere to mandated reporting laws.

Essential Duties and Responsibilities:

  • Provide supervision to assigned staff and monitor the care provided by staff.
  • Maintain knowledge of general health status of all residents of the facility. Monitor vital signs.
  • Perform general assessment of residents as necessary.
  • Respond to emergency calls and needs of the resident; provide direct care as necessary.
  • Assist in transferring residents with mobility limitations in and out of bed, chairs, and wheelchairs; assist resident with transfers to bathroom.
  • Perform administrative duties such as completing health status forms, reports, and other documents as necessary.
  • Complete accident/incident reports as necessary; forward to Resident Care Manager.
  • Report to Resident Care Manager any changes in resident health status.
  • Respond to any concerns or complaints expressed by residents and/or staff and report to Resident Care Manager.
  • Give and receive report at the start and end of each scheduled shift.
  • Provide assistance and administration of resident medications and treatments according to the resident service plan.
  • Ensure residents receive their medication on time.
  • Count and record necessary medications with staff at the beginning and end of each shift.
  • Communicate with pharmacy and reorder resident medications in a timely manner.
  • Utilize Electronic Health Records to communicate with primary care providers.
  • Perform tests such as obtaining urine specimen, International Normalized Ratio, and simple blood glucose monitoring as ordered by provider.
  • Assist residents in getting ready and to their appointments.
  • Ensure results and specimens are recorded or delivered correctly in a timely manner.
  • Perform dressing changes, treatments and other duties as assigned by the primary care provider or other member of the medical team when ordered.
  • Observe and monitor residents vital signs as necessary and at the frequency outlined in the resident service plan or as instructed by primary care provider.
  • Use the department approved electronic charting and Medication Administration Record to completely and accurately chart nursing duties during each shift.
  • Review service plans; ensure that resident care is consistent with resident service plan.
  • Contact the Resident Care Manager/on-call RN, and/or any other emergency personnel in emergency situations by telephone.
  • Contact Resident Care Manager to reorder and maintain direct care staff supplies.
  • Maintain inventory by storing, preparing, and issuing supplies such as gloves and other direct care items.
  • Explain all care to residents and families when appropriate.
  • Communicate with and update families, guardians and members of the medical team about patient health information when appropriate following HIPAA guidelines.
  • Engage in meaningful, culturally competent activities with residents.
  • Guide and supervise assigned staff in correctly following all direct care policies and procedures related to direct care.
  • Honor the residents rights and privacy. Monitor staff compliance of residents rights.
  • Follow all established policies and procedures of the facility.
  • Attend all required trainings and in-services, and any training related to the Saginaw Chippewa Tribal culture and providing culturally sensitive care.
  • Ensure that resident only leave with approved individuals.
  • Adhere to resident confidentiality and comply with department policies and federal regulations under the Privacy rules of the Health Insurance Portability and Accountability Act (HIPAA).
  • Provide excellent customer service for all internal and external customers of the operations at all times. Provide solutions for customer concerns and continually focus on customer service as our top priority.
  • Must maintain strict confidentiality and present a positive, professional demeanor and image at all times.

  • Direct contact with residents, families, and guests in providing care; may encounter emotionally upset residents, family members, or visitors and must be able to handle difficult situations with patience, kindness, and respect.
  • Contact with other health professionals, staff, Elder Service Program, and community resources to exchange information, for collaboration and consultation, and to provide education.
  • Contact with tribal departments providing services to residents.

Knowledge, Skills, and Abilities:

  • Knowledge of applicable federal, state, county and local laws, regulations, and requirements.
  • Knowledge of current principles, methods and procedures for the delivery of nursing procedures and quality of care.
  • Knowledge of prescribed and over the counter medication doses, side effects, interactions for all residents.
  • Knowledge of long term care nursing practices.
  • Knowledge of patient care charts and patient histories.
  • Knowledge of medical terminology and instruments.
  • Knowledge of OSHA guidelines for blood borne pathogens.
  • Knowledge of office functions, procedures, and policies.
  • Skill in treating patients with tact, courtesy, and respect.
  • Skill in operating business computers and office machines, including in a Windows environment, specifically Word, Excel, Access, PowerPoint, and EHR.
  • Ability to lead and guide the work of others.
  • Ability to obtain accurate vital signs.
  • Ability to react quickly and make effective decisions in an emergency situation.
  • Ability to maintain confidentiality.
  • Ability to interact and maintain good working relationships with individuals of varying social and cultural backgrounds.
  • Ability to communicate efficiently and effectively both verbally and in writing.

Physical Demands:

  • Frequent use of hands, wrists, fingers associated with providing medical care and with computer equipment.
  • Required to stand, sit and walk for extended periods of time.
  • Occasionally stoop, kneel, crouch or crawl.
  • Normal visual acuity, ability to talk and hear.
  • Occasionally lift and/or move up to 20 pounds.

Work Environment:

  • Assisted Living Center and Medical clinic.
  • Exposure to bodily fluids such as blood and saliva and/or the potential for needle sticks and/or skin puncture.
  • Latex may be used in the office which can produce allergic reactions.
  • Potential contact with chemicals and/or cleaning reagents utilized within the facility.
  • Exposure to unpleasant odors, radiation, blood-borne pathogens, hazardous material and infectious disease.
  • Situations occur where surgical masks, safety goggles, gloves and protective face shields are needed.
  • Extended hours and irregular shifts may be required.

Job Requirements

  • High School Diploma or GED.
  • Registered or Certified Medical Assistant License.
  • Must be able to pass background check to meet the employment eligibility requirements as they pertain to the position.

Desired Qualifications:

  • One-year experience in long term care or clinic setting.

License, Certification, or Special Requirements:

  • Successful completion of an accredited Medical Assistant program with certification as a Registered/Certified Medical Assistant.
  • Must be able to obtain BLS Certification within probationary period.
  • Must be able to obtain HIPAA certification within five business days from date of hire.
  • Native American preference shall apply to all positions.
