Position SummaryUnder the supervision of the Chief of Health Services, provides supervision, coaching, mentoring and first line support to nurses, medical assistants, care coordinators and other providers on health care service delivery and nursing practice within Saginaw County Community Mental Health Authority (SCCMHA). Supervises and champions our RN-lead, team-based, care management philosophy of care, to provide a health care safety net for all SCCMHA consumers. Functions as a nursing and health care consultant to the agency. Supervises and ensures the provision of health education, health assessments, and general whole-person health care management to consumers with the goal of improving consumer self-management and health outcomes. Supervises patient specific care coordination processes and ensures the efficient use of clinical workflows to achieve consumer specific health outcomes.
This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.
Essential Duties And Responsiblities
  • Supervises service delivery staff within the Health Services Department.
  • Performs annual agency performance evaluations for all reporting subordinate positions and for newly hired subordinates, performs 3- and 6-month evaluations during probationary period and recommends to Chief of Health Services whether employment will continue.
  • Attends meetings: Supervises team huddles, holds regular staff meetings to promote communication and information sharing, other meetings as assigned, represents Health Services at meetings as assigned.
  • Supervises and provides nursing services for consumers using the nursing process and the Scope and Standard of Nursing Practice.
  • Supervises daily performance of the Health Services team members to ensure duties are completed accurately, efficiently, and timely. Monitor and review documentation to meet policy guidelines for evidence-based nursing care, and health care.
  • Interview and onboard new hires in the Health Services Department.
  • Leads the implementation of electronic care coordination platforms, Health Information Technology (HIT) and other tools to identify consumers for focused interventions for risk reduction. (i.e., Azara)
  • Maintains adequate clinical supplies.
  • Ensures clinical equipment remains in operating condition.
  • Maintains, authors, updates, and implements Health Home policies and procedures while ensuring they comply with state, federal and other regulations.
  • Supervises the staff in the Health Services department to maintain workflow, ensuring consumers are seen timely and efficiently.
  • Supervises all risk-mitigation evaluation strategies (REMS).
  • Serves as backup for Health Services staff as needed, and maintains a caseload of consumers, as needed, at the direction of the Chief of Health Services.
  • Participates as a member of the SCCMHA Sentinel Event Review Team to assist in the identification of trends, utilizing this information for process improvement interventions.
  • Participates as a member of the quarterly Leadership Team of SCCMHA Supervisors
  • Participates as a member of the SCCMHA Wellness Committee
  • Supervises and champions innovative processes for care transitions.
  • Acts as liaison in the SCCMHA provider network regarding nursing scope of practice, roles, and internal system relationships.
  • Develops health and wellness initiatives.
  • Acts as a champion of healthy lifestyle changes for consumers and assists in social marketing initiatives encouraging consumer use of Personal Health Records.
  • Applies SCCMHA infection prevention and control policy and procedure throughout the agency and provider network.
  • Communicates the mission, vision, and core values of SCCMHA to staff while holding them accountable and implements these principles in all duties of this position.
Incidental Duties And Responsibilites
  • Ensures that staff are appropriately coached and mentored including recommendations for progressive discipline to the Chief of Health Services, if indicated, is applied equitably to all staff.
  • May address community groups or organizations and represent SCCMHA in community venues such as community population health collaborative(s).
  • May participate in the continuous quality improvement process, attend staff meetings, and consult with other agency programs regarding specific consumer care issues.
  • Attends in-service training, seminars, conferences, and workshops in order to sustain professional competency.
  • Reacts productively to change and handles other essential tasks as assigned.

Supervises :Registered Nurses, Care Coordinators.
Working Conditions/EnvironmentWorks in an office and community settings environment with the business pressures of dealing with time constraints and stress from long periods of computer use. Occasionally operates an automobile to attend appointments in the community and other sites, sometimes during bad weather. Close contact with the public and working with individuals with a developmental disability and/or mental illness who can be dangerous with possible injury to themselves and others.
QualificationsEducation: Associate’s degree in Nursing. Bachelor’s degree in Nursing preferred.
Experience:Graduate of an accredited school of nursing with at least 2-3 years of clinical experience in a medical setting preferred.
Licenses and Certifications:Valid Michigan Driver’s license with a good driving record.
A licensed registered nurse (RN).
Knowledge, Skills, And Abilities
  • Knowledge of scope of the registered nurse, licensed practical nurse, and medical assistants.
  • Knowledge of and ability to apply the standards of nursing practice.
  • Knowledge of and appropriate application of the nursing process.
  • Knowledge and ability to apply the methods, practices, protocols which are Health Home evidence-based practice; the Triple Aim model of health quality improvement; the electronic medical record technology related to decision support and quality management concepts and methods.
  • Promotes a work environment that supports positive work relationships and high team performance.
  • Ability to implement methods and principles of adult education and identify ethical and cultural aspects of situations when making decisions.
  • Strong nursing skills are required and the ability to function as a member of an interdisciplinary team is critical.
  • Effective oral and written communication skills, minimum at college level. Communicates ideas and information verbally and in writing and the ability to prepare documents and presentations in multiple media. The ability to tailor communication to the reader in format and complexity.
  • Computer competency, including EHR, word processing, spreadsheets, databases, internet, E-mail. Ability to exercise independent and mature judgment.
Physical/Mental Requirements:1.Hearing acuity to converse in person and on telephone.
  • Visual Acuity to observe consumers behavior, read and proofread documents and use computers.
  • Ability to walk, stand or sit for extended periods of time.
  • Manual dexterity to write and to operate standard office equipment (PC, Keyboard, Copy Machine, Fax Machine, etc.)
  • Ability to lift and carry files and supplies at least 20 pounds.
  • Strong interpersonal skills to interact with leadership, employees, consumers, and the general public.
  • Analytical skills necessary to conduct research, analyze, and interpret complex data and identify and solve problems by proposing courses of action.
  • Ability to plan short and long range and to manage and schedule time.
  • Ability to handle stress in meeting deadlines and dealing with large numbers of employees and/or consumers.
