Pharmacist Full Time Days Position Summary
The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families.
Responsibilities
This individual’s responsibilities include (but are not limited to):-Order verification and performing medication profile reviews that should include but not limited to: checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency.-Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/ discontinues therapy, and mediates black box warning interventions. -Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin and aminoglycoside consults, etc. -Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders.
Education
Required: Pharm. D. or BS in pharmacy
Experience
Required: 3 years or residency training or equivalent experience
Certification
Required: Pharmacy License; BLS within 30 days of hire; Preferred: Board Certification in Specialty area, CGP, ACLS, etc.
