Saint Luke Health System is Hiring a Case Management Assistant Near Kansas, MO
Shift: Monday – Friday 8:30-5The Work
- Schedules follow up appointment for residents upon discharge
- Facility weekly treatment team meetings
- Coordinates monthly treatment reviews
- Case management duties for residents (including coordination of clothing vouchers, visits, school enrollment, etc)
- Coordinates referrals and admissions to our PRTF program
