The WorkThis is a rewarding and fast-paced position that includes the following responsibilities:
- Identifying and understanding the significant social and emotional factors underlying a resident’s illness.
- Participating with the health care team to complete MDS’s and develop care plans.
- Working with residents and their family members to understand and accept necessary treatment.
- Communicating with insurance companies and obtaining treatment/stay authorizations.
- Helping residents adjust to the temporary and permanent effect of illness.
- Referring residents and family members to community resources when needed.
- Evaluating residents for potential return to home or transfer to a more appropriate level of care.
- Coordinating the resident discharge process.
