CORE JOB FUNCTIONS # Function Approx % of Time Check if Essential 1. ASSESSMENT A. Performs an accurate, ongoing, pertinent assessment for each assigned patient that are age appropriate. Reports/consults pertinent data to other health team members and implements appropriate action. B. Assesses historical data pertinent to medical, social, cultural and educational factors and prior rehabilitation efforts. All areas of the evaluation are addressed. C. Assesses patient/family education needs and learning readiness level, and uses appropriate language and terminology. Repeats instructions often if needed. 15 2. INTERVENTION A. Demonstrates competency in delivery of care based on the patient care plan. Performs nursing skills following Salem Health-West Valley Hospital policies and procedures utilizing appropriate resources. Demonstrates knowledge of age-specific behaviors when caring for patients. Such as: a. Involves family or designated caregiver with care b. Encourage self-care c. Assists patient to plan for anticipated body changes d. Assists, when appropriate, reminiscing about events B. Prioritizes care based on patient’s current needs. Proactive in preventing adverse outcomes. Demonstrates follow-through in the completion of patient care. C. Intervenes appropriately in emergency situations. 15 3. PLAN A. Initiates and/or reviews appropriate plan of care with patient/family to address holistic age-specific needs and reflect the patient’s changing condition. B. Develops a plan of treatment based on the patient’s rehabilitation potential, rehabilitation barriers, objective findings, cultural, social, medical and developmental backgrounds. C. Plans for the transfer/discharge of the patient to the next appropriate level of care in the continuum and is proactive in working with other members of the health care team and/or family as appropriate. 10 4. DOCUMENTATION A. Consistently documents the assessment, planning, interventions and patient responses in a way that accurately reflects the patient’s care and progress. (Admission Assessment, medication management, pain assessment and management, vaccination status, core measures, patient education, plan of care initiation and updates, discharge documentation, etc.) B. Initiates and documents teaching for patient/family, involving other healthcare team members as appropriate. Teaching content and methods based on learning needs, adjusted to appropriate level. 10 5. COMMUNICATION A. Consistently utilizes good communication skills: B. Communicates accurate and complete information in a clear and concise manner (SBAR) C. Anticipates needs and shares information with other team members. D. Provides status updates and verifies information received. 10 6. LEADERSHIP A. Seeks and utilizes evidence based practice 15 B. Actively engaged in continuous professional development C. Maintains good working knowledge of unit based performance and outcomes D. Participates in shared decision making E. Acknowledges and celebrates co-worker’s efforts and achievements F. Assumes the charge nurse role in an effective manner (when assigned): a. Appropriate assignment making, accounting for severity of patient conditions and for staffing mix. b. Supervises and directs care given by others. Assures care delivery is consistent with Standards of Practice. c. Adjusts workload with admissions, procedures, etc. d. Is a resource for staff on duty for collaboration and coordination during the shift. e. Utilizes appropriate problem-solving as issues occur throughout the shift (staff, patient/family concerns, physicians, etc.) f. Notifies appropriate team members of potential problems g. Completes administrative tasks such as: unit based activities and outcomes and performance improvement activities, newsletter, surveys or joining various committees 7. SAFETY A. Assists in maintaining a safe unit environment for patients, staff, and self, i.e. replacing SHARP’s containers, following infection control policies, taking broken equipment out of service, using proper lifting and transfer techniques, keeping the hallways free of clutter/spills, etc. B. Uses patient restraints appropriately, safely, and documents same (if applicable). 15 8. RESOURCE MANAGEMENT A. Demonstrates ability and willingness to assist management staff in full utilization of their skills. Flexibility, cooperative effort, and time management are demonstrated on a continual basis through a willingness to be cross-trained to other areas of service. B. Maintains adequate supplies and reordering as necessary to ensure smooth operations. C. Accepts a floating shift when needed to maintain unit productivity. 10 DEPARTMENT/AREA SPECIFIC FUNCTIONS: Refer to department specific functions and expectations. SUPERVISORY RELATIONSHIPS: 1. Provides clinical oversight for (if applicable): Licensed Practical Nurses (LPNs), Certified Nurse Assistants (CNAs), Clinical Tech, & other Non-licensed staff, Unit Clerks, Health Professional Students.

Background/Expertise:

QUALIFICATIONS: Type Core Job Qualifications PositionSpecific Qualifications Education Experienced RNs (Internal/External): • BSN or non-nursing Bachelor’s Degree with MSN for all external applicants for Salem Hospital, preferred for West Valley Hospital. • ADN graduate. Experienced ADN’s with less than 5 years of acute care experience required to sign agreement to complete BSN or MSN within 3 years of hire. • Graduate of a Diploma RN Program and at least 5 years of acute care RN experience. • Note: For employees who were hired as an ADN prior to October 2011, there is no requirement to obtain BSN. New Grad RNs: • BSN or MSN. • ADN new graduate required to sign agreement to complete BSN or MSN within three (3) years of hire. Experience • Minimum of one (1) year of RN Staff Nurse experience required, acute care RN Staff Nurse experience preferred. • For candidates with less than one (1) year, see New Grad requirements. Licenses/ Certification/ Registration • Current unencumbered licensure to practice nursing as a Registered Nurse in the State of Oregon. • BLS issued by the American Heart Association required. • ACLS is required for ICU & CVCU within 90- days of hire.
