HOSPICE CONTINUOUS CARE LVN - Legacy Hospice and Palliative Care, located in Chino, CA, is currently looking for compassionate and caring LVN's to assist with shift care for Continuous Care assignments to manage crisis symptoms. The areas of coverage are Whittier, Chino, Rancho and Anaheim/Placentia and the surrounding areas.

We are a hospice and palliative care agency seeking LVN's to do per-diem crisis care and symptoms management shift work to address patient and family concerns, and to work with their team to ensure communication of important information and care plan changes. Must have working knowledge of HospiceMD. Come be a part of our TEAM!!!

LICENSED VOCATIONAL NURSE: Continuous Care can range from $35.00 to $40.00 per hour.
