The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of multiple divisions - the San Francisco Health Network, Population Health, Behavioral Health Services, and Administration. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers. The Population Health Division (PHD) provides core public health services for the City and County of San Francisco: health protection, health promotion, disease and injury prevention, and disaster preparedness and response. Behavioral Health Services operates in conjunction with SFHN and provides a range of mental health and substance use treatment services.

Role description

Under the direction of and reporting to the Director of the Transitional Age Youth System of Care (TAY SOC), the Assistant Director is responsible for managing the clinical infrastructure and implementation of initiatives to ensure that the clinical operations, practices, policies, and quality improvement goals across the TAY SOC are met. The Assistant Director oversees key functions, including clinical operations, quality assurance, utilization management, the transition of Electronic Health Records (EHR), and the implementation of Epic.
The Assistant Director, Transitional Age Youth System of Care (0922 Manager I) performs the following essential job functions:
Manages the clinical operations for the TAY System of Care; ensures the implementation and tracking of operational policies and changes, including the transition from Avatar to Epic, CalAIM (e.g., clinical documentation, LOE, PURQC), managed care plans, utilization management, BHS regulatory compliance, and contracted programs as part of ongoing development of TAY System of Care clinical infrastructure
Serves as Epic lead for the TAY system, ensuring adherence to the BHS operational guide across all BHS programs in Epic and Carelink, with a focus on centralized referral access and workflows; triages, troubleshoots, and addresses clinical operational process issues and technical needs related to Epic including charge reconciliation errors, level of effort, billing and CPT codes; serves as TAY-SME for BHS Clinical Operations Guide
Provides clinical consultation, training, mentoring, coaching, and other technical assistance to providers across the TAY specialty and non-specialty service continuum, as well as offering consultation to various systems serving transitional age youth with behavioral health needs; supports the Director in strengthening new capacity-building initiatives for providers
Ensures the TAY Linkage Hub operates in alignment as a Representative of Office of Coordinated Care (R-OCC); collaborates closely at a systems level to coordinate and ensure effective linkage, consultation, and care coordination with the TAY System of Care, Office of Coordinated Care (OCC), and all systems serving Transitional Age Youth, including service delivery partners such as the criminal justice system, BHS Children Youth and Families (CYF) System of Care, BHS Adult/Older Adult (AOA) System of Care, primary care, the Human Services Agency, and the Department of Homelessness and Supportive Housing
Manages and troubleshoots clinical system issues when triaging and navigating complex behavioral health client referrals across TAY serving systems and diverse service delivery partners
Reviews and manages complex/high acuity case requests or submissions including facilitation of targeted case conferencing, or redirection to appropriate system’s risk level meetings, or navigating linkage pathways within and/or outside of BHS system
Supervises and provides clinical supervision, including coaching, monitoring performance, and conducting annual performance evaluations
Facilitates TAY System Care Coordination (TSCC) meetings with clinical program representatives and system partners to address system barriers and support coordination of care and resource connections across programs
Manages all quality improvement efforts by assisting in the evaluation of procedures, maintaining organized statistical records, monitoring services/deliverables, reviewing system data with TAY SOC leadership, identifying issues, and proposing new, relevant procedures to the TAY Director
Fosters collaboration and coordination among sub-units including various sections of Behavioral Health Services such as IT/Electronic Health Records, Billing/Medi-Cal Revenue Optimization, Compliance, Quality Management, BOCC, Contracts, and Facilities/Operations; participates in the coordination and planning of service delivery systems, problem-solving, and policy development of assigned modalities of care within the System of Care
Participates in and implements various racial equity initiatives that enhance our system of care, including Culturally Congruent efforts and/or other activities in alignment with the BHS and TAY SOC racial equity plan and objectives
The Assistant Director, Transitional Age Youth System of Care (0922 Manager I) may perform other duties as assigned/required.

How to qualify

Required Minimum Qualifications
License: Possession of a valid license issued by the California Board of Behavioral Sciences (BBS) as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), OR Licensed Professional Clinical Counselor (LPCC); AND
Experience : Three (3) years of professional experience overseeing, monitoring, coordinating, and/or managing a program providing behavioral health services.
One year full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

Desirable Qualifications: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring:
Three (3) years of direct experience serving underserved Transitional Age Youth populations

Candidates that meet the minimum qualifications will be invited to participate in a Training and Experience evaluation that is designed to measure the knowledge, skills, and abilities in job related areas which may include but not be limited to: Knowledge of evidence-based practices, standardized methods, and procedures and techniques of clinical psychology including diagnosis, prevention, and treatment of psychological distress, mental disorders, and substance use disorders of individuals and groups, especially transitional age youth and those at risk of or experiencing homelessness, clinical skills in coordinating care, communications, and services tailored to effectively meet the needs of clients with behavioral health issues, clinical skills in triaging behavioral health client referrals across transitional age youth-serving systems, including client screening, assessment and intakes, provider consultation, information gathering, and troubleshooting complexities, skill in qualitative and quantitative data analysis, electronic health record systems, quality improvement models and tools, and utilization of adult learning theory and training methodologies, interpersonal and communication skills – accessible and easy to work with, strength-focused, and detail-oriented, ability to collaborate and build consensus with every section and units within the organization, across city agencies, hospitals, systems, and community partners, and ability to provide clinical supervision in a behavioral health setting and direct subordinates in performing diverse activities.
