Position Purpose:
As a Social Work Consultant at Santa Fe Care Center, you will have the unique opportunity to make a meaningful impact on the long-term care industry. We are seeking a dedicated professional to join our team in a Part time capacity. allowing you to work directly with clients in various healthcare settings.
Essential Functions of Position:
- Conduct assessments, evaluations, and provide consulting services in the field of social work.
- Develop customized strategies and positive outcome plans to enhance the quality of care in client facilities.
- Collaborate with facility staff and management to address social work-related challenges.
- Provide guidance and support to clients to ensure compliance with industry regulations and standards.
- Maintain accurate documentation and records of consulting activities.
Education / Requirements:
- Licensed Social Worker in New Mexico.
- Minimum of 1-2 years of experience in social work, preferably in a healthcare or long-term care setting.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a collaborative team.
