- $2 per hour NOC Shift Differential
- $3 hour Weekend Shift Differential (Day & NOC)

10 hour shifts
Job SummaryA Licensed Practical Nurse (LPN) working in our Skilled Nursing facility is responsible for the daily nursing care needs of our residents. This position is a valuable clinical team member contributing to the smooth, organized and efficient operations of the nursing department.
Responsibilities
- Provide primary nursing care to residents with varying backgrounds and medical histories
- Assists with new admissions
- A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider
- Provide medical intervention as needed for emergent needs
- Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc.
- As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members
- Implement and follow Primary Care Providers orders
- Other Duties as Assigned
