Education: Must have bachelors degree in related field. Will be required to maintain certifications in CPR and CPI.
Work experience: Must have experience working in the mental health and substance abuse field preferred, but not required.
Knowledge, Skills & Abilities: Must possess good verbal and oral communication skills, problem solving skills and ability to multitask. Must be computer literate. Ability to complete assignments accurately, in a timely manner and within procedural guidelines. Organizes and prioritizes work.
Interpersonal Skills: Possess excellent interpersonal skills to help facilitate the development of rapport with patients and their family members, referral sources; and to work cooperatively with in-house physicians, and administrative, clinical and support staff members. Essential Job Duties

Performs direct and indirect patient case management duties to assist patients in maintaining recovery, personal dignity, and safety.
- Facilitate patient admissions through conducting telephone interviews and completion of admission paperwork including consents, agreements, and financial obligations forms once patient arrives
- Coordinate case management issues with other treatment team members to be documented in treatment plans
- Coordinate appointments and transportation and aftercare needs of patients before, during, and following treatment
- Meet with patients to discuss insurance and help facilitate any changes that need to happen to help the patient.
- Provide ongoing support and case manager expertise to patients to help improve quality of life.

Participate as an active member of the treatment team in the development, implementation, and evaluation of patients treatment plan from admission through discharge.
- Assist Therapist and Discharge Coordinator with discharge planning and community resources.
- Attend treatment team and staffing meetings to report on patient discharge planning.
- Contribute to treatment planning process i.e. goals and objectives with Therapist.
- Facilitate and document educational lecture groups in conjunction with other treatment team members.

Participate in the Performance Improvement process.
- Review and comply with P&Ps.

Serve as the Case Manager to assist patients with but not limited to:
- Financial obligations while in treatment.
- Communicating with family/payees.
- Facilitating monthly budgets.
- Insurance issues.
- Social Security issues.
- Assisting patients in obtaining needed documents patients need (i.e., Drivers License, Social Security card, etc.).
- Assist with accessing further educational needs after discharge.
- Working with community resources and patient to increase patient care.
