Job Type Full-timeDescriptionWe are currently seeking a highly motivated LPN, RN, or LMSW case manager to join our team. The Spine Hospital of Louisiana is a 23-bed specialty hospital owned and operated by a dedicated team of local physicians committed to providing world-class spinal care to our patients. In this role, you will support and guide patients and families throughout their discharge process. You will work closely with patients to assess their needs and make appropriate referrals regarding their post-op needs.Responsibilities : Conduct assessments to determine patient needs and develop care plansProvide ongoing support and guidance to patients and their familiesConnect patients with appropriate resources and facilitate referrals for post-acute careMonitor patient progress and conduct daily clinical reviewsCollaborate with other healthcare professionals to ensure comprehensive care for patients Requirements Minimum of a diploma in practical nursing, an associate degree in nursing, or a master's degree in social workCurrent LMSW, LPN, or RN licenseMinimum of 5 years of experience in case managementStrong communication and interpersonal skillsAbility to work independently and as part of a teamKnowledge of community resources and servicesAbility to maintain confidentiality and professionalism at all times If you are passionate about helping others and have the skills and experience we are looking for, we encourage you to apply for this exciting opportunity.
