We are seeking a compassionate and skilled Case Manager to join our team. In this role, you will be responsible for coordinating client care, ensuring that each individual receives tailored support to achieve their highest level of functioning. This position is vital in helping our clients navigate their healthcare needs effectively.
Responsibilities
- Assess client needs and develop individualized care plans.
- Coordinate services and resources for clients, ensuring comprehensive support.
- Provide crisis intervention and management as needed.
- Conduct regular follow-ups to monitor client progress and adjust care plans accordingly.
- Collaborate with healthcare providers, family members, and community resources to enhance client care.
- Utilize motivational interviewing techniques to empower clients in their care journey.
- Document all client interactions and maintain accurate records of services provided.
Requirements
- Bachelor's degree in social work or a related field preferred.
- Experience in case management or social work is highly desirable.
- Strong crisis intervention and management skills.
- Knowledge of Medicare and other healthcare systems is a plus.
- Excellent communication and interpersonal skills for effective collaboration with clients and stakeholders.
- Ability to work independently while managing multiple cases efficiently.
- Reassessments/Assessments, treatment plans and discharge summaries.
