At Seasoned Recruitment, we are seeking a compassionate and dedicated PTA to provide quality home health care to patients in their own homes. As a Home Health Physical Therapist Assistant, you will work closely with a team of healthcare professionals to deliver personalized treatment plans and help patients regain their mobility and independence.

Responsibilities:

• Assist in providing physical therapy services to patients in their homes
• Collaborate with the physical therapist to develop and implement individualized treatment plans
• Guide and support patients through therapeutic exercises and activities
• Monitor and record patient progress during therapy sessions
• Educate patients and their families on proper techniques for exercises and activities
• Assist with modalities and other treatment interventions as directed by the physical therapist
• Maintain accurate and timely documentation of patient treatment and progress
• Communicate effectively with the healthcare team to ensure coordinated and comprehensive care

Requirements

Qualifications:

• Graduation from an accredited Physical Therapist Assistant program
• Licensure or certification as a Physical Therapist Assistant in the relevant state
• Prior experience in home health or a related setting is preferred
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Compassionate and patient-centered approach to care
• Valid driver's license and reliable transportation
