Position Summary: This position will provide supportive and medical social services to terminally ill patients and their families in accordance with an appropriate plan of care. The Social Worker will assume responsibility for the development, implementation, and management of psycho-social services provided to hospice patients and families in the home care or residential setting.
Qualifications Include:
Bachelor degree in Social Work from an accredited Social Work Program required. MSW preferred.
Licensed Social Worker license in Minnesota
Two years SW experience within the last five years preferred; Hospice or Homecare experience preferred.
Valid driver’s license with acceptable driving record per policy
Daily access to a reliable and insured vehicle. Must have strong written and verbal communication skills, strong organizational skills, accurate and attentive to details, proficient keyboarding skills, prior experience with electronic medical record systems and belief in the hospice concept.
