Seattle Children's is Hiring a Director, Care Coordination Near Seattle, WA

ResponsibilitiesThe Director of Care Coordination is responsible for the development of a strategic plan, priorities, and objectives by aligning and adapting daily operations to achieve organizational expectations for service excellence, quality, and financial outcomes. The Director is accountable for appropriate and effective use of resources, productivity outcomes, and adherence to social work and case management professional practice standards while adapting and maintaining daily operations for areas of oversight. Accountable for collaborating on, and implementing, initiatives aligned with organizational objectives and priorities to establish effective care coordination across the continuum of care. In partnership with the Medical Director, Care Coordination is responsible for appropriate clinically focused outcome measures, quality improvement activities, and care coordination across the across the continuum of care. The Director, Care Coordination develops and maintains effective relationships with leaders from across the organization including the Foundation by cultivating and sustaining a culture that embraces and exemplifies professional practice, the development of future leaders, and leadership excellence.
RequirementsRequired Education/Experience:
  • Bachelor's degree in clinical, financial, or management discipline.
  • Master’s degree in Nursing or Social Work. (Candidates currently enrolled in a Master’s program will be considered. Individual must complete program within three years in position and maintain solid performance).
  • Minimum five (5) years of leadership experience in a health care setting.
Required Credentials
  • Current Washington State social work or RN license.
  • Must obtain national certification in a clinical specialty area or leadership within three years.
  • If RN, a copy of graduate degree in Nursing or transcripts indicating attainment of Master's degree in nursing is to be on record with Human Resources upon hire. If Master's degree is in Business Administration, Public Health, Healthcare Administration, or related field, a copy of the degrees or transcripts indicating attainment of a Master's degree and Bachelor's in Nursing degree is to be on record with Human Resources upon hire. If Master's degree is in process, a copy of the Bachelor's degree in Nursing or transcripts indicating attainment of a Bachelor's degree in Nursing and a notation of expected Master's completion date is to be on file with Human Resources upon hire.
  • Minimum three years’ experience in utilization management, case management, social work programs, discharge planning or other cost/quality management program.
Preferred
  • Three (3) years’ experience in hospital and ambulatory based care coordination programs.
  • Knowledge of leading practice in clinical care and payor requirements.
  • Self-motivated, proven communication skills, proactive.
  • Background in business planning, and targeted outcomes.
  • Working knowledge of managed care, inpatient, outpatient, and the home health continuum, as well as utilization management and case management (both discharge planning and social work).
  • Working knowledge of the concepts associated with Performance Improvement.
  • Demonstrated effective working relationship with interdisciplinary team and external partners.
Min to Max Hourly Salary: $71.61 - $107.41 /hr Min to Max Annual Salary: $148,948.80 - $223,412.80 /yr Salary Information:
This compensation range was calculated based on full-time employment (2080 hours worked per calendar year). Offers are determined by multiple factors including equity, skills, experience, and expertise, and may vary within the range provided.
