LPN Job Description

We are looking for a dedicated Licensed Practical Nurse to join our team. As an LPN, you will be responsible for overseeing all services required to satisfy the resident service plan, including medical / clinical aspects of care.

You will work directly with the Administrator to ensure goals and objectives are met, while providing top-quality service to each resident.

Qualifications :

• Must have relevant education and experience working with frail elderly and those with dementia.
• LPN license preferred.
• Experience in gerontology or geriatrics.

Responsibilities :

• Oversee medication administration and proper documentation of medical records.
• Coordinate services with outside providers for individual resident needs.
• Ensure communication with family members, physicians, and therapists as needed.
• Monitor resident records and conduct safety reviews regularly.
