SUMMARY This rolecombines tasks from both Care Partner and Medication Technician positions,involving a mix of hands-on care and assistance with medication. The exactresponsibilities will depend on the community's needs. Assists residents withActivities of Daily Living (ADLs). Work with residents living withdementia, Alzheimer's, and/or memory-impaired, with chemical dependency,or similar issues under the direction of the Care Manager/Medication Manager. Responsiblefor the accurate and timely distribution of medications to residents accordingto the written orders and instructions provided by physicians. This positionalso involves following all medication practices per Medication Training,Company Policy, and Title 22 Regulations ESSENTIAL DUTIES AND RESPONSIBILITIES include thefollowing. Other duties may be assigned by your direct manager. Regular andpredictable attendance is an essential function of the job.
Assist residents with Activities ofDaily Living (ADLs) including, but not limited to continence care, showering,grooming, transfer assistance, and meal assistance
Ensure that all residents alwaysremain clean, dignified, and presentable
During your shift, utilize acompany-provided phone to access care plans and chart care accordingly
Follow a care plan and documentprovided care accordingly
Reports should occur by the end ofthe shift and should be documented in the changeover notes unless immediateintervention is needed
Observe Resident's Rights, asposted within the facility. Promptly respond to resident calls for assistance
Always know the location anddisposition of all residents (under direct care) during shift
Pour and assist residents withmedications according to Physician orders, Medication Training, and Companypolicies and procedures
Follow "Medication Training"policies and procedures regarding infection control, standard precautions,biohazard waste, sharps management and disposal
Fax new orders to pharmacyfollowing Company policies and procedures
Communicate to Physician,Pharmacist, Family, and Direct Supervisor if resident misses any scheduleddoses of medication
Document immediately whenmedication is given to resident
Re-order medications followingCompany policies and procedures
Observe and immediately report anychanges in resident condition following the "Medication Training" policies andprocedures
Document immediately when aresident receives a PRN medication
Keep medication room and cart cleanand orderly at all times
Complete all documentation on a newresident for the medication program
Log in all medication deliveries onthe resident's Centrally Stored sheet
Keep medication room, cart,cupboards, and records locked when not in use
Destroy medications followingCompany policies and procedures
Count the narcotics followingCompany policies and procedures
File appropriate paperwork inresident's charts
Assist Medication Manager withreviewing current Medication Assistance Records (MAR's) to the next month'sMAR's for any changes
Assist Medication Manager withinvestigation on any concerns from resident or family regarding medications
Verify the resident's medicationsprior to dispensation with the resident's physician and every six monthsthereafter in order to provide the best service and make sure the resident istaking the proper dosages and the proper medication
Ensure residents do not run out ofmedications at any time
Record any medication changes perdoctor's orders in all the proper documents as specified in MedicationProcedures and Company Policies
In both roles, understands allemergency protocols, responds to emergencies as directed by Supervisor and perCommunity training. Treat residents with dignity and respect and provideprivacy during personal care
QUALIFICATIONS EDUCATION and/or EXPERIENCE
Minimumof high school education or GED or combination of related experience
Experience is preferred, but notnecessary
Reasonable expectations ofmathematical, technical, and reasoning skills to perform essential jobresponsibilities
According to Title 22 requirements,employees must be at least eighteen (18) years of age
CERTIFICATES,LICENSES, REGISTRATIONS
Must be authorized to work in theUnited States of America and provide sufficient work authorizationdocumentation(s)
DOJ and FBI fingerprint clearancemust be completed before working in the community (obtained through thecommunity)
Health screening/Physical andnegative TB test must be less than six (6) months old (obtained through thecommunity)
First Aid/CPR Certification(obtained through the community)
