It is an innovative model of care for seniors 55 years of age and older who are eligible for Medicare and Medical Assistance or able to pay privately, and who are medically eligible. We enable seniors to maintain both th...eir quality of life and their independence by providing a wide array of support services designed to help them address their personal, physical and social needs. We serve the healthcare needs of medically eligible seniors living at home.
