Full Time: 40 hours per week

Annual salary of $78,000

Provide comprehensive social services to meet the emotional and social needs of residents at the time of move-in, during their residence, and if necessary when they depart the community.
Act as a referral source directing participants and their families to community social service providers, and serves as a liaison with family members, community agencies, physicians, and care providers.
Administer state Medicaid programs and must be familiar with additional programs for seniors, such as Medicare and the Veteran's Aid and Attendance Benefit.

REQUIREMENTS:

License: Graduate of an accredited college with a valid license as a LSW, LCSW, LICSW, or LSWA
Experience: 1 years working with elderly populations, and experience with regulations of Assisted Living. Knowledge of state financial programs such as Medicaid, a MUST!
Communication: An ability to communicate effectively with residents and co-workers
Punctuality & reliability – Our residents rely on us to assist them with their daily living activities and we expect our associates to deliver on this promise
