Sentara is Hiring a Care Coordinator, Population Health - Hybrid Near Virginia, VA

Sentara Health is seeking to hire a qualified individual to join our team as a Care Coordinator, Population Health.

Position Status: Full-time, Days

Standard Working Hours: Monday - Friday, 8am - 5:00pm (EST)

This position is Hybrid at either Sentara Park in Virginia Beach, VA or off site near Sentara Martha Jefferson Hospital in Charlottesville, VA or off site at Harrisonburg, VA. 2 days in office and 3 days from home.

Minimum Requirements:

  • Associate's level degree - In lieu of the degree requirement, five years direct patient education experience will be considered or LPN or Medical Assistant certification or CPhT with PTCB certification.

  • Patient Care - 2 years required

  • Population Health experience preferred

Job Description:

As a key member of the Population Health Team, the Care Coordinator will contribute to results as defined by the principles of value-based care. The position will collaborate with Population Health RNs or Clinical Pharmacy Specialists, providers and practice staff to engage in long-term management approaches and optimize treatment for patients with targeted conditions to achieve desired health outcomes. Responsibilities will include implementation of patient care plans with RN or Clinical Pharmacy Specialist supervision and to monitor patient progress.
