​Sentara Health is now hiring a Registered Nurse Program Coordinator. This position will oversee admissions for the three Inpatient Rehabilitation Facilities in Norfolk and surrounding areas.

Education/Experience requirements: Must have a BSN degree with 3 years of clinical experience. Experience working in Program Development and working with Physicians are strongly desired.

Duties/Responsibilities

Provides clinical and administrative leadership in the development and implementation of a comprehensive Clinical Program and the relevant patient population. Responsible for the development, monitoring, and implementation of quality outcomes and collaborates with multidisciplinary team in outcomes management process. Directs and guides patient and family education and the activities of care team members. The position oversees data collection through chart review, computerized statistics and patient follow-up for performance improvement activities and prepares reports. Has overall responsibility for management of program certifications and regulatory requirements.
