The Program Coordinator position is responsible for ensuring the development, implementation, coordination, and monitoring of ISP's, Behavioral and/or Therapeutic Treatment Plans for individuals on their caseload within the Sponsor program.

Responsibilities:

  • Perform monthly home inspections/visits of Sponsored Residential Homes
  • Document and create reports (incident reports, annuals, etc.)
  • Coordinate and conduct various meetings (intakes)
  • Maintain current and accurate files on caseload (collect and review monthly documentation)
  • Develop and maintain relationships with individual's family members/guardian(s), social worker's, CSB's, etc.
  • Assess individual needs for recommendations and input (verbal/written) regarding issues of admission, transfers, and discharges
  • Ensure adherence to licensure (Human Rights, State/Federal/Local Regulations)
  • Review support data and progress notes (support plans, etc.)

Qualifications

  • 1 year of experience providing direct services with adults diagnosed of an intellectual disability or other developmental disabilities
  • 1 year experience interpreting and implementing federal, state, or local regulations (DBHDS)
  • Bachelor's degree in the Human Service's field (Sociology, Social Work, Special Education, Psychology, etc.)
  • Experience with Therap's (preferred)
  • Experience with WAMS (preferred)
  • Excellent verbal and written communication
  • Ability to work closely with clients
  • Interpersonal skills
  • In-depth knowledge of incident reporting
  • An understanding of DBHDS and DMAS
