About Serenity, Inc.

The goal of Serenity is to provide skills building and risk reduction counseling to decrease the spread of HIV. Serenity provides HIV-positive clients, aged 16 and older and their families with case management & skills training to live productive and full lives while reducing the risk of transmission of HIV.

The HOPWA Case Manager is responsible for providing housing navigation, case management, and financial assistance to persons living with HIV/AIDS. A HUD funded program administered by the City of Richmond, HOPWA requirements and procedures may change from year to year. The HOPWA Case Manager is expected to remain current with those changes and conduct their activities in compliance with them.

The HOPWA Case Manager is responsible for:

• Responding to initial client contacts within 48 hours
• Completing a housing intake on all clients requesting HOPWA assistance
• Providing Housing Navigation to assist clients in identifying housing needs, locating housing options, and accessing the chosen housing site
• Ensuring that all required HOPWA client documentation is received, accurate, and complete
• Conducting HOPWA Housing Habitability Standards and Lead Abatement inspections
• Completing Rent Reasonableness evaluations and other HOPWA documentation requirements
• Providing regular HOPWA case management to all HOPWA clients at least once a month, more frequently as needed
• Reviewing HOPWA files with the Executive Director on a weekly basis
• Entering HOPWA client information into HCIS on a weekly basis
• Submitting written HOPWA funding requests to the Executive Director for approval
• Preparing quarterly and annual HOPWA reports
• Other duties as assigned by the Executive Director

Job Types: Full-time, Part-time

Pay: $33,000.00 per year

Expected hours: 30 – 40 per week
