This is a hybrid position is responsible for the day- to-day operations of Shared Living/ Adult Family Care supports in assigned clients, with the goal of providing quality care and physical well being of individuals within the Seven Hills Family Services, Inc.
As a Case Manager, you will work with a team of clinical, nursing and other professionals, oversee and monitor the Coordination of Shared Living and Adult Family Care supports with assigned clients and assure compliance with State, Federal and other regulatory agencies. Our Case Managers work in cooperation with the team members, funding sources, families and contracted Shared Living Caregivers.
Qualifications
BA required with two (2) years experience with individuals with disabilities OR Seniors (work related, family or volunteer)
Valid Driver's License & COVID-19 vaccination required.
Essential Functions
Provide case management services and coordinate all related support services in relation to needs of identified individuals.
Some travel is required, our office location in Sturbridge two days per week, and remote and in-person home visits to surrounding towns in our service area as required by program.
In this role, you will collaborate with Outreach/Recruiter and Assistant Area Director in the qualification process of Shared Living/AFC Caregivers, to include: screening and interviewing of potential providers/ complete intakes for individuals referred and the completion of all required documentation.
Working with the SHFS team of Clinicians, Nurses and Supervisors you will maintain accurate and up to date records relative to consumers and providers in all Shared Living/AFC supports in assigned area.
Facilitate and schedule meetings of funding agencies, family members and clinical team members as needed.
Maintain a personal connection and accessible to families, management and funders in areas relevant to the assigned work.
Responsible for the daily operation of assigned work in accordance with all policies and procedures set forth by Seven Hills Family Services, Seven Hills Foundation, and all federal and/or state licensing agency regulations which Seven Hills Family Services is required to follow.
Oversee members allocated supports respite and supports as designated in contracts.
In conjunction with Area Director, to unerstand and coordinate adjunt supports services for individuals supported and to ensure accurate tracking of these supports.
To coordinate and ensure that all Shared Living/AFC Providers are properly trained and oversee that ongoing training occurs as deemed necessary.
To attend specific job related trainings and current certifications as required.
You will meet regularly with the Area Director for support and supervision and discuss visits of each Shared Living/AFC home as required and assure through surveys/visits that each individual is receiving appropriate supports.
