SGA YOUTH & FAMILY SERVICES NFP is Hiring a FAM TEAM FACILITATOR- CCN Near Homewood, IL

Description

Annual Salary $45,000

The Family Team Facilitator collaborates with youth, families, and informal/formal supports to identify individualized needs for youth dually-involved in the Child Welfare and Juvenile Justice systems and develop a plan of care to meet those needs. Facilitators should display effective inter-personal skills, a dedication to helping youth and families reach their goals, and a strong attention to detail in the coordination of services and care.

MAJOR DUTIES AND RESPONSIBILITES:

  • Manage and maintain a maximum caseload of 13 youth, ages 11-19, whom are in DCFS care and justiceinvolved.
  • Display effective inter-personal skills, a dedication to helping youth and families reach their goals, and a strong attention to detail in the coordination of services and care.
  • Collaborate with youth, families, and informal/formal supports to identify individualized needs for youth dually-involved in the Child Welfare and Juvenile Justice systems and develop a plan of care to meet those needs.
  • Utilize effective interpersonal skills to engage youth, their support system, child welfare Caseworker, and
  • other service providers in an introductory meeting, youth meetings and ongoing family team meetings.
  • Complete a strength-based assessment of all life domains identifying both strengths and needs
  • Facilitate and lead the Family Team in all meetings, ensuring the youth and family voice is heard and respected by all team members.
  • In collaboration with the Family Team, develops a comprehensive plan of care, assigning responsibilities to each team member, to address needs including both non-traditional services and evidence-based treatments, as appropriate.
  • Guide the team to adjust the plan to address any anticipated crisis situation.
  • Provide consultation and support to the Family Team in the implementation of the plan of care and
  • monitoring the progress toward goal achievement.
  • Advocate for the youth and family, especially around meeting their needs in the least restrictive setting possible.
  • Maintain all administrative tasks in a timely manner through Google Drive, ETO case notes (the data management system) and on-site case file.
  • Adhere to the fidelity of Wraparound facilitation, as taught during on-the-job training.
  • Home visits and meetings with youth/families within their community (Cook or Lake County) is required.
  • Engage in ongoing professional development and maintain up to date knowledge of DCFS policies.
  • Other duties as assigned.

Requirements

Education:

  • Bachelor’s Degree from a Counseling, Criminal Justice, Social Work, or related program, preferred

Experience:

  • Experience coordinating care and/or working with youth and families on a direct service level.
  • Basic knowledge of the Illinois child welfare and juvenile justice systems; knowledge of local resources and service providers is preferred.
  • Experience working with diverse populations and in diverse communities Other:
  • Strong collaborative, relationship building, and communication skills to effectively resolve Problems, achieve goals, and meet deliverables.
  • Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups.
  • Assertive personality that is willing to follow up with non-engaged youth, family, and supports to ensure tasks are completed in a timely manner.
  • Attention to detail and thoroughness. ? Proficient computer skills in Microsoft Windows and internet proficiency, including the use of Google Drive applications. ? Effective verbal and written communication skills.
  • Ability to exercise discretion and ensure confidentiality. 

KEY COMPETENCIES:

  • Ability to work varying hours, including evenings & weekends, to accommodate family’s needs.
  • Must have a vehicle with valid driver’s license and automobile insurance required.

PHYSICAL REQUIREMENTS:

  • Must be able to remain stationary 50% of the time.
  • Must be able to frequently move inside the office to access file cabinets, office machinery, etc. ? Must be able to transport oneself to and from home visits.
  • Occasionally ascends/descends stairs.
  • Constantly moves to assist during program activities.
  • At times may be required to lift up to 25 lbs. for various needs.
  • Frequently moves in outdoor weather conditions.
