The Care Coordinator (nurse) will work on quality care initiatives and value-based activities for the health system. Additionally, the care coordinator will perform patient outreach to coordinate all care needs and resources. The care coordinator must have excellent communication abilities and enjoy working with a dynamic collaborative team.

Minimum Education and Experience Requirements::
Licensed Nurse in Florida.
BSN degree.
1-3 years of nursing experience.
Care Coordination experience required.
