Job Summary |
Ensures patients receive care, treatment and services that are timely, appropriate, and evidence based. This is accomplished through collaboration with the multidisciplinary care team throughout the continuum of care and through performance improvement activities. Collaboratively develops and implements disease management protocols/guidelines that reflect the most current research and practices. Facilitates gathering data and reporting accurate clinical documentation through extensive concurrent interactions with providers and nursing staff. Supports timely, accurate and complete documentation of clinical information used for measuring and reporting outcomes. Serves as an ongoing resource person for providers, nursing staff, managers and ancillary departments. Identifies, develops, and participates in hospital and community programs related to stroke.
