Job Title: Registered Nurse

Location: Baltimore, MD

Rate: $60-65/hr

Job Overview:

We are looking for a compassionate and detail-oriented Registered Nurse to join our team in Baltimore, MD. The successful candidate will be responsible for evaluating the needs of clients through comprehensive assessments, developing personalized care plans, and connecting clients with necessary resources and services. This role involves conducting home visits and working closely with other professionals to ensure clients receive the support they need to maintain their independence.

Key Responsibilities:

• Conduct Comprehensive Assessments: Analyze medical and psychosocial information, perform home visits, and evaluate client needs to determine suitable services.
• Develop Personalized Care Plans: Assist clients in accessing home and community-based services that support their independence in the least restrictive environment possible.
• Coordinate and Connect: Refer clients to appropriate resources, consult with other professionals, and coordinate services to create a seamless support system.
• Champion Client Autonomy: Advocate for client rights, promote self-determination, and ensure fair access to essential services.
• Document for Impact: Maintain accurate records to track client progress, inform service decisions, and contribute to the ongoing enhancement of the AERS program.

Qualifications:

• Holds a valid State of Maryland RN license.
• Minimum of 2 years of experience in medical-surgical, geriatric, and/or pediatric case management.
• Ability to pass a comprehensive background investigation.
• Reliable transportation and a valid driver’s license equivalent to a noncommercial, class C Maryland driver’s license.
• Willingness to travel to client homes throughout Baltimore City, Maryland.
