This position is funded by a grant for a duration of time as defined by the grantor.
This position is responsible for coordinating and contributing to the success of the lead poisoning prevention program, which aims to improve the health and housing of families with children under the age of six (6). Provides care coordination efforts by planning and executing patient follow-up procedures, as well as working directly with families that have lead-prisoned children or are enrolled in the local Housing HUD Lead Hazard Reduction grants.Pay Grade 51
