Shelby Residential and Vocational Services (SRVS) is Hiring a PROGRAM SPECIALIST Near Memphis, TN

Job Purpose (or Description): The Program Specialist (PS) is responsible for providing overall comprehensive supports to ensure that the people being supported thrive through coordination of medical, social, financial, religious, recreational, housing, employment, and day services. The Program Specialist will ensure that staff supports are in place for the person supported to live as full participants in his/her home and community utilizing a person-centered approach to develop, implement and monitor the Individual Support Plan (ISP) and Person-Centered Support Plan (PCSP), while focusing on the environment, health, safety and wellbeing of the person supported.

Duties and Responsibilities:
  • Ensures the program is delivered in an appropriate, supportive, professional, and respectful manner.
  • Ensures that support is provided according to all relevant policies, procedures, and
regulations.
  • Ensures that Assistant Program Coordinators (APC) and Direct Support Professionals (DSP) are trained to administer the required support.
  • Monitors and evaluates all tasks of the APCs and DSPs assigned to the Program Specialist's caseload.
  • Monitors supplies and resources of the person supported and agency.
  • Works closely with admissions and discharges to identify persons requiring
community/supported living placement and needs of persons supported.
  • Makes recommendations for changes and improvements to the program, as appropriate.
  • Schedules work assignments with regard to DSP training, experience, and while monitoring overtime.
  • Discusses any concerns with the supervisor.
  • Ensures that all persons supported have received adequate and sufficient services.
  • Coordinates appropriate support and equipment including other community resources, as required.
  • Establishes and maintains a current, accurate, and confidential reporting system for persons supported.
  • Provides information to other health care professionals, as required and appropriate.
  • Consults with family members and other natural supports to ensure that support is on-going, and that all person supported needs are identified and met.
  • Provides person supported with identified needs in the home and community.
  • Identifies and manages home care needs including general housekeeping, personal hygiene and any unanticipated events or unstable situations.
  • Coordinates services for medical appointments and activities.
  • Ensures medical instructions and plans are carefully and consistently followed.
  • Coordinates community resources and supports.
  • Establishes healthy relationships with families, if applicable and appropriate.
  • Notifies and educates families on medical issues and other resources, as applicable and appropriate.
  • Establishes and maintains current, accurate, confidential files for each person served.
  • Educates person served, families and support systems about the Program Services.
  • Represents self as agency liaison between the person supported, staff, family, or legal representative, management, co-workers, and external customers.
  • Performs other duties as assigned.
Non-Essential Job Functions:
  • Attends all scheduled and mandatory meetings.
  • Serves on agency committees, as assigned.
  • Works cooperatively with supervisors, staff members, families, monitors, and others.
  • Demonstrates respect for all persons served.
  • Performs other duties as assigned.

Scope of Position:

The Program Specialist provides and coordinates individualized person-centered supports and resources to people with disabilities, so that they can age in place enjoying the opportunity to live as independently as possible in their homes and in the community they have selected. This position must conduct in-depth assessments of the person supported needs, advocate, develop, and monitor progress by keeping thorough records and help to develop a detailed action plan to meet the needs of the person supported, including their medical, home and daily needs.

Assistance may include, but is not limited to transportation, staff training, house cleaning, personal hygiene, providing nutritious meals, job coaching as appropriate, medical appointments and follow up to recommendations, medication assistance, and staffing to promote the health and wellness related activities for the person supported.

The Program Specialist is also responsible for managing and administering medications to person served, as appropriate and with proper training.

The Program Specialist will maintain confidential files. Budget, and program information of the persons served. The Program Specialist will prepare and present reports and information as required.

The Program Specialist will manage and distribute resources effectively and will ensure that all support is provided in a caring and respectful way in keeping with all relevant SRVS policies and procedures and governing agencies to promote best practice and standards.

The Program Specialist will serve as a liaison with other community services and resources to ensure that other resources are involved, if required.

The Program Specialist will also provide information and education to individuals and groups in the community, as required and requested.

Failure to provide adequate supports and services may put the people supported and others at risk and/or force the person supported to move from their homes before they are prepared to do so.

Position Type/Expected Hours of Work:
This is a full-time position. Days of work are Monday through Friday. This position will occasionally require long hours and weekend work.

Location: Clarksville, TN area

Salary: $48,000

Must be 18 years or older. Must have a valid TN or KY drivers' license. One year of experience in the Direct Support care or working DD population preferred. At least one year of supervisory or managerial experience preferred. Must be able to pass all required pre-employment screenings, criminal background checks, and a drug test.
