Sheppard Pratt is Hiring a School-Based Case Manager (Howard County) Near Columbia, MD

General Summary:Provide culturally and linguistically appropriate and comprehensive case management services for the school-based mental health in collaboration with managing department. The services are to promote self-sufficiency skills, family development, and linkages to mental health resources which are targeted to youth and their families. Collaborate with the clinical treatment team, school staff, and community partners in providing comprehensive care while maintaining strict confidentiality of client information. Provide early intervention and prevention services to students who are identified by therapists, counselors, and other school-based staff.
Principal Responsibilities and Duties:
  • Provide case management services to students/parents/families with economic and/or social needs such as housing, healthcare, employment assistance, food, clothing, education, and safety needs per established policies.
  • Use designated electronic records system to document services provided to clients and their families.
  • Must enter data into the system within 48 hours from the date that the services were delivered.
  • Manage required data collection.
  • Will respond to phone calls and crisis services of potential participants.
  • Provide compliance with contract terms and agency expectations as defined by the program leadership.
  • Inform treatment team of progress on case management activities and discharge planning.
  • Develop database of case management resources.
  • Organizes, coordinates, and facilitates a wide variety of meetings, workshops, presentations, groups, activities, and events to develop partnerships between families and schools.
  • Work closely with service providers and develop a referral system to promote a continuum of care.
  • May conduct home visit meetings as necessary and/or make referrals, as necessary.
  • Participates in workshops and conferences as requested by immediate supervisor.
  • Attend regular staff meetings and supervision as scheduled.
  • Travel as needed to various program sites or to sites of community stakeholders.
  • Performs all other direct job-related duties as is assigned.
Knowledge, Skills, and Abilities Required:
  • Requires a BA/BS in social work, Counseling, Psychology, or related mental health or social service-related field and 1 year of prior experience serving parents/families in need (internship experience considered). Prior experience can also be substituted for each year of required education, in addition to the standard experience requirement listed above.
  • Knowledge and understanding of mental health conditions and person-centered approach.
  • Prior experience/equivalent knowledge of immigrant populations & acculturation issues preferred.
  • Knowledge of community resources in the service area assigned is preferred or willingness to learn and master.
  • Good organizational, verbal, and written communication skills, and ability to multi-task.
  • Basic computer skills to include knowledge of Word & Excel.
  • Must have access to a vehicle to transport self between work locations.
  • Bilingual capabilities preferred (English/Spanish).

  • This position is school and community based and requires frequent travel between programs and services which may include homes and school sites
  • Work in environments where there may be potential exposure to communicable disease.
  • Ability to maintain moderate physical activity within the community to include walking (including stairs), lifting, and driving.
  • Flexibility in working hours may be needed to coordinate direct care and services with families.
