This position will provide direct mental health services to adults who meet the criteria for "serious and persistent mental illness". Services include, crisis intervention, case management, and treatment planning. This position will involve development and implementation of person-centered treatment plans with individuals, their families and support networks.
These examples of typical duties do not include all possible tasks for this classification and do not limit the assignment of related tasks. Regular attendance according to the department's approved work schedule is required for all Sherburne County positions.
Education: BA/BS in Social Work or closely related field.
Must have a valid MN driver's license and reliable vehicle.
Experience:
Social Worker I: Minimum of completed Social Work internship or closely related field
Social Worker II: Minimum of 1 year of Social Work experience or equivalent education plus work experience
Preferred Experience: Experience working with adults with Severe and Persistent Mental Illness; Adult Mental Health Rule 79 Case Management training; Crisis Intervention Training; and dual diagnosis training. Licensure by the State of Minnesota as a Licensed Alcohol and Drug Counselor (LADC), Licensed Independent Clinical Social Worker (LICSW), or Licensed Social Worker (LSW). Training in SSIS.
