Sholom Community Alliance is Hiring a Director of Home Care Services Near Park, MN

SUMMARY

The Director of Home Care Services is responsible for the Comprehensive Home Care operations of an assigned site including clients, staff and the coordination of Home Care and housing interactions.

PRIMARY RESPONSIBILITIES

1) Supervise the home care operations of an assigned site including clients, staff and the coordination of Home Care and housing interactions. 

a. Ensure compliance with Minnesota Comprehensive Home Care Rules and statutes and with agency policies and procedures. 

2) Observe budgetary guidelines and provide accurate reporting of expenses, coding and timely submission through proper channels. Assist with billing of services as needed. 

3) Supervise staff and participate in hiring and selection, recognition and counseling as needed. 

a. Define staffing ratios based on client acuity and service requirements.

b. Ensure staff education regarding Minnesota Comprehensive Home Care Rule and Statute requirements.

c. Develop appropriate communication tools for staff to ensure best quality care for clients.

4) Complete initial and regular assessments of clients to assure their needs will be met, adhering to the Nurse Practice Act. 

a. Communicate to physicians and other appropriate personnel (e.g., county caseworkers, families, and responsible parties) changes in client needs or conditions.

b. Facilitate coordination of client services with other service providers.

5) Ensure clients maintain admission criteria; reassess and recommend alternative placement if necessary. 

6) Identify and respond appropriately to emergency situations.

a. Monitor for and address any safety hazards that exist in staff or clients’ habits and activities of daily living and in the building environment.

7) Perform miscellaneous projects and completes various tasks as requested by management. 

8) Ensure proper billing practices and procedures.

9) Other duties and responsibilities as assigned.

JOB SPECIFICATIONS

Education, Experience and Credentials

  • Must be licensed as a Registered Nurse in the State of Minnesota, or the ability to procure within a specified period of time, and must maintain licensure.
  • Two or more years of experience in a similar position and in a similar care environment preferred.
  • One-year previous related experience in a supervisory position and in a similar care environment highly preferred.
  • Current certification in CPR preferred; upon hire if not certified, must obtain no later than 60 days from hire.

Knowledge, Skills and Abilities

  • Flexible and productive in an environment with regular interruptions.
  • Good general nursing skills and proficiency in homecare nursing. Interest in and desire to work with older adults with understanding and compassion. Initiative and judgment in determining the needs of residents and maintaining patient care delivery in conformance with recognized standards.
  • Able to adhere to the Homecare Bill of Rights, HIPAA, and the Nurse Practice Act.
  • Able to keep current on changes in the industry, including clinical and regulatory changes.
  • Able to comply with established Infection Control and Safety Guidelines.
  • Able to work as an effective and proactive team-player; understands the importance of supporting the organization, residents/families/customers and other Sholom employees.
  • Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.
  • Excellent interpersonal skills to interact effectively with a variety of people and personalities inside and outside the organization. Able to effectively handle interactions at all levels and to respond appropriately in sensitive situations.
  • Possess effective problem solving skills.
  • Able to understand and follow facility and department safety and emergency procedures.
  • Strong computer skills, including Microsoft Word, Outlook, Internet, and other applications. Must be skilled in database management and record keeping.

PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB

  • Perform light work, with occasional or negligible lifting of objects weighing up to 20 pounds.
  • Communicate clearly and effectively both orally and in writing, including possessing good English speaking skills, fluency and understandability. 
  • Able to effectively use office equipment to review and create information.
  • Work at a desk for extended periods of time and perform long hours of work on a computer. 
  • Frequently stand and walk to move around office/building repeatedly throughout the day.
  • Able to work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.
  • Able to obtain reliable transportation.

TOOLS AND EQUIPMENT USED

  • Use thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment.
  • Use telephone, computer and standard office equipment. 

WORK ENVIRONMENT

The work environment is primarily indoors. There is some exposure to the outdoors and inclement weather when traveling between sites.
