Sholom Community Alliance is Hiring a Program Manager - Memory Care Near Paul, MN

SUMMARY

The Program Manager is responsible for leading, managing and supporting the Memory Care team (residents, family and staff) in promoting a person centered approach to operations that will ensure the residents live the life they prefer.

PRIMARY RESPONSIBILITIES

1. Oversee the day to day operations of the Memory Care household, in partnership with the RN Coordinator and Director of Homecare Services. * 

a. Supervise the home care operations of an assigned site including clients and staff. *

b. Ensure compliance with applicable state, federal, and agency policies and procedures. *

c. Participate in hiring and selection, recognition and counseling of staff as needed. *

2. Facilitate the planning and implementation of strategies and programming that ensure the highest quality service and quality of life. *

a. Ensure coordination of client services with other service providers. *

b. Participate in weekly Therapeutic Recreation meetings. *

3. Maintain comprehensive documentation and reports per policy and regulation. *

a. Ensure the completion of all required documentation on the computer or on forms in an accurate and timely manner according to policies and procedures. *

4. Identify and respond appropriately to emergency situations. *

a. Monitor for and address any safety hazards that exist in staff or clients’ habits and activities of daily living and in the building environment. *

5. Assist with various other projects and tasks as needed.

JOB SPECIFICATIONS

Education, Experience and Credentials

· Three to five years’ experience in older adult services. 

· Education, experience or licensure in geriatric social work, activities or gerontology preferred. 

· Management/leadership experience preferred. 

· Successful completion of (or ability to become certified within six months of hire) a state approved nursing assistant training program and current certification in state(s) in which he/she practices OR successful completion of a home health aide instruction courses and successful completion of home health aide competency testing to work in Home Care. 

· Current certification in CPR preferred; upon hire if not certified, must obtain no later than 60 days from hire.

Knowledge, Skills and Abilities

· Flexible and productive in an environment with regular interruptions.

· Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.

· Possess effective problem solving skills.

· Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications. Must be skilled in database management and record keeping.

PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB

Incumbent must be able to:

· Perform heavy work, lifting up to 50 pounds, and frequently lifting/carrying of objects weighing up to 20 pounds as needed.

· Push/pull residents in wheelchairs up to 200 pounds a distance of several feet as needed

· Effectively communicate, including following instructions, in order to communicate with people inside and outside the organization.

· Able to effectively use office equipment to review and create information.

· Work at a desk for extended periods of time and perform long hours of work sitting at a computer. 

· Frequently stand and walk to move around office/building repeatedly throughout the day.

· Able to work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.

· Able to obtain reliable transportation

TOOLS AND EQUIPMENT USED

  • Use telephone, computer and standard office equipment. 

WORK ENVIRONMENT

The work environment is primarily indoors. There is some exposure to the outdoors and inclement weather when traveling between sites. May be at risk of exposure to bodily fluids.
