SHS SWEDISH HEALTH SERVICES is Hiring a Navigator RN – Center for Healthy Aging (Hybrid) Near Seattle, WA
DescriptionThe RN navigator serves as a consistent resource to support optimal care delivery for patients and other projects as assigned, focusing on coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up. The nurse navigator is knowledgeable of multiple treatment decisions, evidence-based standards, best practice guidelines and maintains care collaboratively across the continuum. In collaboration with clinicians, identifies and facilitates optimum care delivery with clinical expertise, strong interpersonal abilities and critical thinking skills. Provides effective oral or written feedback to providers to identify care gaps.
