Siffrin is Hiring a Quality Improvement Manager - Adult Day Services Near Canton, OH

Position Summary:

To improve the quality and capacity of service delivery of Adult Day Services (ADS) at all Siffrin locations as evidenced by an increase in the achievement of individual outcomes that include the development of skills that lead to greater independence, community membership, relationship building and self-direction, as well as self-advocacy; improved staff capacity to support individuals with complex needs; and increased staff understanding and focus on person-centered outcomes, individualized goals and skill attainment.

Essential Functions:

  • Researches and assures continuous implementation of rules and recommendations for standards for ADS programs provided by DODD.
  • Uses self-assessment tools for ADS providers to document quality standard and licensing compliance in Siffrin’s ADS programming and fulfillment of the Capacity Improvement Plan as described in the ADS Quality Pilot Program Application.
  • Provides technical assistance and resources to ADS staff and managers to improve substandard quality and licensing deficiencies through professional development and quality improvement plans.
  • Communicates training/education opportunities to ADS staff and monitors completion of required trainings.
  • Provides follow-up technical assistance to ADS staff on topics from assigned trainings to assure understanding and implementation in the ADS setting.
  • Participates in relevant state and local workgroups as assigned for educational purposes and to provide stakeholder feedback as required for the ADS Pilot Program and/or the state’s and Siffrin’s commitment to continuous quality improvement.
  • Collects, monitors, and shares data as part of a continuous quality improvement measure.
  • Travels to each ADS site to conduct in-person services as well as use technology to hold and/or attend meetings.
  • Provides ongoing communications to parents/guardians, SSAs, DODD staff, internal and external stakeholders on program and individual updates.
  • Establishes and maintains positive working relationships with inter and intra-agency staff, clients, parents, guardians and stakeholders.
  • Assists in implementation and monitoring of up-to-date resources, internal controls, and outcome metrics specific to increasing and/or assessing the effectiveness of the department.
  • Participates in at least 5 hours of on-going or special training per year.
  • Completes other related duties as required and/or assigned.
  • Follows the Siffrin, Inc. Code of Ethics in the fulfillment of duties.

Physical and Environmental Requirements

In order to perform required duties to carry, move or lift supplies and materials; work with computers, copiers, equipment and supplies: the employee must be able to stand, walk, sit, talk and hear, use hands to manipulate, type, handle or feel; stoop, kneel, crouch and reach with hands and arms.

Work is conducted at ADS sites. Drives to and from other work sites for meetings, in-service, or training may be required. The employee may be subject to adverse weather conditions when driving.

Qualifications:

A High School Diploma or GED is required. A bachelor’s degree in a related field or equivalent experience is preferred.

Must meet the agency’s guidelines with respect to a criminal background check.

Must have basic computer skills including Word, Outlook, and internet-based knowledge as well as how to navigate and complete basic tasks.
