We are currently seeking a Part-Time Palliative Care Nurse Practitioner in South LA areas who shares our passion for providing dignified, compassionate end-of-life care. 

Part B Palliative admissions and care management experience preferred.

Shift Schedule: 3 days a week (flexible on which days, 8AM-5PM)

Same competitive hourly rate for all patient care, meetings, training and drive time mileage reimbursement.

The non-negotiables:

• Graduate of an accredited school of Advanced Practice Registered Nursing
• End of Life Nursing Education Program (ELNEC) – to include all 8 modules must be completed within 18 months of employment with Silverado Hospice
• Current Advanced Practice Registered Nurse License within the state of employment
• MSN or Doctoral Degree required
• Certification in Hospice and Palliative Nursing desired but not required
• Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient’s location
• Must be able to lift 50 pounds
• Must clear a criminal background check, physical and drug screening

What you'll be doing:

• Performs comprehensive nursing assessments
• Assesses signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
• Initiates plan of care, all services provided to patients/families in all settings
• Provides direct Face to Face visits to referred and existing patients contacts physician for patient updates
• Maintains knowledge of state and federal regulations and JCAHO standards (if applicable)
• Participates in QAPI program
