The Dementia Care Specialist (DCS) is a multifaceted position that requires intimate knowledge and skills in working with individuals at risk for or who have been diagnosed with Alzheimer's Disease and Related Disorders (ADRD), and those with Intellectual and Developmental Disabilities. The DCS uses a systematic approach that is influenced by evidence-based practice, behavioral symptom management education, and interventions to aid in caregiver stress relief.

  •  Competent and knowledgeable regarding Alzheimer's Disease and Related Disorders (ADRD), disease trajectory, its symptoms, treatments, co-existing conditions, etc. 
  • Familiar with community-based resources available through Medi-Cal HCBS, other HCBS
  • Uses appropriate assessment skills for evaluation of the social and emotional responses of the client, family/caregiver to chronic illness and caregiving
  • Demonstrates knowledge and awareness of community resources available to clients and their families/caregivers, and assists with referrals and linkages
  • Communicates effectively with social and community agencies regarding supportive care cases and the psychosocial needs of the client, family/caregiver, especially for those with high-risk indicators, i.e.: abuse/neglect, inadequate food/medical supplies, and high suicide potential
  • Build sustainable relationships with targeted referral sources by maintaining regular contact with appropriate personnel and ensuring good rapport
  • Maintains productivity standards established by the agency
  • Documents notes of each client visit reflecting client evaluation, intervention, client/family/caregiver response
  • Participates in the orientation, training, and in-servicing of staff and volunteers
  • Communicates unresolved work-related problems using appropriate chain of command, consistently demonstrates effective problem-solving skills, handles confidential information appropriately, and participates in team building activities
  • Demonstrates positive behavior characteristics such as attitude, initiative, cooperation, adaptability, fairness, ingenuity, and dependability
  • Demonstrates ability to work with a variety of belief systems, and honors individual choices and lifestyles
  • Demonstrates flexibility, punctuality, and attendance reliability with changing workload/caseload; sets priorities accordingly and carries out job assignments in a self-directed manner
  • Responsible for verifying cell phone is adequately charged and operational at all times
  • Other duties as delegated by the Chief Clinical Officer or Designee

Requirements

  • Bachelor’s degree preferably in the health or human services-related field; a master’s degree (preferred)* 
  • At least one year of full-time paid experience working directly with multiple people with dementia and family or informal caregivers (for example, providing direct care, caregiver support, support group facilitation, residential care management, home care)* 
  • Expertise and knowledge regarding older adults, dementia, dementia care and support, family and informal caregiver needs, and challenging behaviors 
  • Excellent communication skills, demonstrated skills in assisting individuals and families in problem-solving and utilizing community resources
  • Skills and experience in facilitating small group learning and discussion programs 
  • Experience with family caregivers of people with dementia and family dynamics
  • Experience in developing collaborative relationships, preferably with community organizations and healthcare professionals
  • Competence in public speaking and engaging community members and professionally
  • Ability to provide culturally competent services and supports 
  • Ability to function independently and be resourceful in the roles undertaken 
  • Ability to adjust work hours to meet with program participants and to provide community and employer outreach and educational programs
  • Basic computer skills, including the ability to record and report program data and information
  • Must be comfortable driving in the foothills of Georgetown, Pollock Pines, Grizzly Flats 
  • Valid California driver’s license and the ability to travel for community events and to make home visits
